What should I bring to registration?

New Players

Your child's birth certificate to show proof of age. (We just need to see a copy; we don't need to keep it.)

All Players

....Your child's height and weight and/or clothing size. Uniforms will be available to try on at registration. The uniform needs to be a little big to account for growth over the winter and to allow for clothing underneath the uniform during cold weather. It is better for the uniform to be a little big than a little small. Please DO NOT GUESS! If we order your child's uniform based on a guess and you guess incorrectly, we will NOT be able to exchange it for another size.

....Parent's information including name, address, home phone number, email address, place of business, business address, and business phone number.

.... Emergency contact information including the name and phone number of the person you would like us to call if there is an injury during a practice or game and you are not available. Your child's doctor's name and phone number. Your insurance information for secondary AYSO insurance purposes. Your child's school information. A list of any disabilities your child has including hearing, sight, asthma, allergies (especially bee stings and food allergies), ADD, ADHD, etc. This information helps the coach help your child have a good time.

. The standard age divisions for the Boys and Girls programs are as follows:

U-5 ...Under five (5) years of age and not younger than 4 years of age.

U-6... Under six (6) years of age -

U-8 ...Under eight (8) years of age

U-10 .Under ten (10) years of age

U-12 .Under twelve (12) years of age

U-14. Under fourteen (14) years of age

U-16. Under sixteen (16) years of age

U-19. Under nineteen (19) years of age

...The effective date of age determination shall be the player's age as of July 31st of the year that the season commences. Players who have reached the age of four years and six months as of this date are also eligible to participate in the program.

May 17, 10am to 2pm at Sports Complex

May 31, 10:00am to 2:00pm at Sports Complex

 

Pre-register

 

: 1. Click the logo below (it will take you to E-AYSO) and pre-register your player(s).

..2. Once your pre-registration is complete, you will be able to print out your personalized registration forms. Make sure you print off TWO copies of these forms.

..3. Once you make the copies of the registration bring the copies to one of the registrations Look on the Calendar link to see the dates and places
